Supporting Texas Families: Delfina, TACHC, and Legacy Community Health Collaboration

Alongside Dr. Erica Giwa, Medical Director of OB/GYN at Legacy Community Health, the Delfina team participated in the Texas Association of Community Health Centers (TACHC) webinar, Emerging Innovations in Maternal Health. The session focused on the current maternal health landscape in Texas and demonstrated how technology-enabled, proactive care is being implemented at Legacy to address the maternal health crisis.

The discussion also highlighted how community health providers can utilize innovative tools, like Delfina, to improve patient support and maternal health outcomes. A key feature of the webinar was the case study of Mrs. B, a patient whose postpartum complications were caught early through Delfina remote monitoring. Learn more about Mrs. B’s story here

Case Study: Mrs. B

Mrs. B experienced an uncomplicated pregnancy with no signs of high blood pressure. However, four days after delivery, her care team was alerted to a dangerous blood pressure spike via readings she tracked in the Delfina app. This early identification allowed her provider to immediately refer her back to the hospital for necessary care.
